Neostigmine methylsulfate
Indications/Uses
Myasthenia gravis, antagonist to non-depolarizing neuromuscular blockade, paralytic ileus, post-op distension, urinary retention, paroxysmal supraventricular tachycardia.
Dosage/Direction for Use
Post-op intestinal atony Prophylaxis: 250 mcg. Treatment: 500 mcg SC/IM/slow IV repeated at 4-5 hr interval. Urinary retention Prophylaxis: 250 mcg. Treatment: 500 mcg SC/IM. Continue 0.5 mg at 3 hr interval for at least 5 additional inj. Myasthenia gravis 1 mg IM every hr.
Contraindications
Hypersensitivity. Peritonitis, mechanical intestinal or urinary tract obstruction or doubtful bowel viability.
Special Precautions
Epilepsy, bronchial asthma, bradycardia, recent coronary occlusion, vagotonia, hyperthyroidism, cardiac arrhythmias, peptic ulcer. When large doses are given, simultaneous administration of atropine sulfate is advisable. Hypotension & bradycardia if the effect of gallamine of curare is antagonized.
Adverse Reactions
Nausea, vomiting, diarrhea, abdominal cramps, increased salivation & bronchial secretions, miosis, diaphoresis.  Muscle cramps, fasciculation, weakness.
Drug Interactions
Muscle relaxants eg, suxamethonium. Cyclopropane or halothane anesth. Prolonged bradycardia w/ β-adrenoceptor blocking agents. Antagonized effects w/ aminoglycosides & some anti-arrhythmics eg, quinidine.
